What are the responsibilities and job description for the Part-Time/PRN Hospice RN case manager position at Fremont Regional Hospice?
The Case Manager plays a vital role in providing Medical care and support to individuals and families facing life challenges, including illness, grief, and loss. This position requires a compassionate and skilled individual who can offer guidance, comfort, and medical guidance to those in need. The Case Manager will work collaboratively with healthcare teams to ensure holistic care is provided, addressing the medical needs of patients.

This is a Full-Time position. Monday thru Friday days 8 AM-4 PM. Plus some on-call
Responsibilities:
- Complete initial and ongoing comprehensive assessments of patients and families
- Initiate patient plan of care and reevaluate as necessary
- utilize health assessment data to determine nursing diagnosis
- Administer medication and treatment as prescribed by the physician in the plan of care
- Counsel the patient and family in meeting nursing and related needs
- Complete and maintain accurate clinical notes
- Communicate with the physician and participate in interdisciplinary group meetings to meet the medical and emotional needs of the patient and family/caregivers
- Ensure compliance with all state and federal legal and regulatory requirements
- Additional responsibilities as assigned
- Qualifications:
- Graduate of an accredited nursing school with current licensure to practice in the state of operation
- 2 experience preferred
- Experience in hospice or a similar setting preferred
- Knowledge of the hospice philosophy of care
- Commitment to clinical and documentation excellence
