Fresenius Medical Care seeks a skilled Master Social Worker to provide psychosocial services to patients with chronic kidney disease. As a Dialysis Psychosocial Specialist, you will work collaboratively with healthcare teams to support patient rehabilitation and improve quality of life.
Key Responsibilities:
- Participate in care planning to identify effective interventions for patient treatment goals
- Provide monitoring and interventions for patients to adjust to dialysis and achieve optimal psychosocial status
- Assist patients with referral to community resources, including home health services and vocational rehabilitation
- Document interactions and interventions provided to patients and/or families
Requirements:
- Masters in Social Work degree
- Knowledge of social work theory and methods
- Excellent communication and interpersonal skills
