Key Responsibilities
- Establish and maintain relationships with healthcare professionals, social workers, and community resources to promote referrals and collaboration.
- Develop and disseminate educational materials, workshops, and training programs to enhance healthcare provider knowledge and skills in addressing psychosocial aspects of kidney disease.
- Participate in interdisciplinary meetings, committees, and working groups to advance shared goals and objectives.
- Monitor and analyze data to identify trends, patterns, and areas for improvement, informing strategic initiatives and program development.
Requirements
- Master's degree in Social Work (MSW) from an accredited institution.
- State-issued license or certification as a Social Worker, with 2 years of experience in a healthcare or social services setting.
- Excellent communication, interpersonal, and project management skills.
