As a Clinical Social Worker - ESRD, you will work with the healthcare team to provide psychosocial services to patients treated by the facility.
Purpose and Scope:
- The Clinical Social Worker - ESRD will assess patients' psychosocial status, strengths, and areas of need that may affect rehabilitation and optimal treatment outcomes as part of the comprehensive patient assessment.
- In collaboration with the interdisciplinary team, the Clinical Social Worker - ESRD will participate in care planning to identify effective interventions that will help the patient meet rehabilitation, treatment goals, and improve quality of life.
- The Clinical Social Worker - ESRD will utilize FKC patient education programs, established social work theory, and methods to address barriers and meet patient treatment goals.
Responsibilities:
- The Clinical Social Worker - ESRD will provide monitoring and interventions for the patient to adjust to dialysis and achieve optimal psychosocial status and quality of life.
- The Clinical Social Worker - ESRD will provide supportive counseling services to patients as permitted within the scope of their clinical training and state license.
- The Clinical Social Worker - ESRD will collaborate with the interdisciplinary team on transplant waitlist management and facilitate the transplant referral process.
Requirements:
- A Master's degree in Social Work (MSW) is required.
- The Clinical Social Worker - ESRD must have state required licensure and meet the applicable scope of practice board and licensure requirements in effect in the State in which they are employed.
- No prior experience is required, but 0-2 years' related experience is preferred.
