Clinical Social Worker Specialist

Job Description: We are seeking a highly motivated and compassionate Clinical Social Worker to join our team. In this role, you will work closely with our healthcare team to provide psychosocial services for our dialysis clinic patients. This will include assessing patients' psychosocial status, strengths, and areas of need, and collaborating with them to identify effective interventions that promote positive adjustment, rehabilitation, and improved quality of life.

Responsibilities:

• Assess patients' psychosocial status, strengths, and areas of need
• Collaborate with patients and healthcare team to identify effective interventions
• Provide supportive counseling services to patients as permitted within the scope of clinical training and state license
• Facilitate discussions of advance directive wishes with healthcare team and patient's family/support persons
• Report on quality indicators related to adherence
• Work with patients, families, and healthcare team to provide education tailored to their learning style, communication barriers, and needs
• Review patient rights and responsibilities, grievance information, and other facilities policies with patients and/or their representatives
• Provide training to clinic staff pertaining to psychosocial topics as needed

Requirements:

• Masters in Social Work
