Key Responsibilities
- Participate in care planning with patients and the healthcare team to identify effective interventions that address barriers to treatment and improve quality of life.
- Utilize FKC patient education programs, social work theory, and methods to develop personalized care plans and provide supportive counseling services.
- Maintain current knowledge of local vocational/educational rehabilitation programs and assist patients with referrals and access to resources.
- Facilitate discussions regarding advance directive wishes with the healthcare team and patients' families, as needed.
