Job Description
\Fresenius Medical Care North America is seeking a skilled and compassionate Social Worker to join our team. As a Social Worker, you will play a vital role in supporting patients with end-stage renal disease (ESRD) and their families. Your primary responsibility will be to provide psychosocial services to patients undergoing dialysis treatment, promoting positive adjustment, rehabilitation, and improved quality of life.
\Your Key Responsibilities

- Assess patients' psychosocial status, identifying strengths and areas of need that may affect treatment outcomes. \
- Participate in care planning, collaborating with the healthcare team to develop effective interventions that help patients meet rehabilitation goals and improve quality of life. \
- Provide established outcomes-driven psycho-educational counseling methods aimed at improving treatment adherence, vocational/educational rehabilitation, and/or quality of life. \
- Support patients who are seeking transplant, providing informative and goal-directed counseling. \
- Maintain current knowledge regarding local vocational/educational rehabilitation programs, assisting patients with referral and access to these resources. \
- Collaborate with the healthcare team to promote patient understanding and informed decision-making regarding advance directives and DNR status. \
- Provide usual care and/or Social Work Intensive to address non-adherence and quality of life concerns for all patients. \
- Develop and maintain effective relationships through timely communication, ensuring outstanding customer service to all external and internal customers. \
- Collaborate with facility management, Financial Coordinators, and Billing Group staff to address patient issues related to insurance, payment, and financial assistance. \
- Understand FMCNA financial policies and procedures, educating patients about available programs and resolving billing concerns. \
- Assist with interview process and decision to hire new personnel if requested by CM or AM/DO. \
- Develop and provide training to staff on psychosocial topics as needed, including Ultra Care Programs. \
- Other duties as assigned. \
- Travel required (if multiple facilities). \
- Knowledgeable of and adheres to FMCNA Social Work Policy, including documentation. \
- Masters In Social Work Required. \
- State Specific Licensure required. \
- 0 - 2 years' related experience. \
- Hourly Rate: $22 - $36. \
