About the Job
We are looking for a dedicated and empathetic Healthcare Advocate to join our team. In this role, you will provide supportive counseling services to patients, including monitoring and interventions for patients to adjust to dialysis and achieve optimal psychosocial status and quality of life.
You will also provide educational and goal-directed counseling to patients who are seeking transplant and facilitate discussions regarding advance directives, DNR status, and other relevant issues.
