Job Title: Psychosocial Services Specialist
About the Role:
Fresenius Medical Care is seeking a skilled Psychosocial Services Specialist to provide critical support to our patients and their families. As a vital member of our interdisciplinary team, you will utilize your expertise in social work theory and practice to promote positive adjustment, rehabilitation, and improved quality of life for our patients.
Key Responsibilities:
- Participate in care planning with patients and healthcare teams to identify effective interventions that enhance patient rehabilitation and treatment goals.
- Utilize FKC patient education programs, established social work theory, and quality of life measurement instruments to address barriers and meet patient treatment goals.
- Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life.
- Offer supportive counseling services to patients as permitted within the scope of clinical training and state licensure.
- Maintain current knowledge of local vocational/educational rehabilitation programs and assist patients with referrals and access to these resources.
Requirements:
- Masters in Social Work degree
- State licensure or certification as a social worker
- Excellent communication and interpersonal skills
- Able to maintain confidentiality and handle sensitive patient information
- Ability to travel between facilities (if required)
