Key Responsibilities
- Assess patients' psychosocial status, strengths, and areas of need that may affect rehabilitation and optimal treatment outcomes.
- Participate in care planning in collaboration with the patient and healthcare team to identify effective interventions that will help the patient meet rehabilitation, treatment goals, and improve quality of life.
- Provide monitoring and interventions for the patient to adjust to dialysis and achieve optimal psychosocial status and quality of life.
- Support patients in accessing community resources, such as home health services, vocational rehabilitation, and other essential services.
Requirements
- Masters in Social Work degree from an accredited institution.
- State required license to practice as a social worker.
- Meets the applicable scope of practice board and licensure requirements in effect in the State in which they are employed.
Work Environment
The ideal candidate will have 0-2 years of related experience and excellent communication skills. They will be working in a fast-paced environment with a dynamic team. The successful candidate will have a strong understanding of kidney disease and its impact on patients and their families.
