The Clinical Social Worker Role: As a Clinical Social Worker at Fresenius Medical Care, you will have the opportunity to make a meaningful impact on the lives of patients with kidney disease. You will work collaboratively with our healthcare team to provide individualized psychosocial support, which may include:
- Assessing patients' psychosocial status, strengths, and areas of need
- Collaborating with patients and healthcare team to develop and implement individualized plans of care
- Providing supportive counseling services to patients as permitted within the scope of clinical training and state license
- Facilitating discussions of advance directive wishes with healthcare team and patient's family/support persons
- Monitoring and reporting on quality indicators related to adherence
- Developing and delivering educational programs tailored to patients' learning styles, communication barriers, and needs
- Training clinic staff on psychosocial topics as needed
