Job Summary:
Fresenius Medical Care is seeking a highly motivated and empathetic Patient Advocate and Support Coordinator to join our team. This dynamic role involves providing exceptional patient care and support to individuals undergoing dialysis treatment.
Responsibilities:
- Assess patients' psychosocial status, strengths, and areas of need to inform care planning
- Develop and implement individualized care plans to address patients' unique needs and goals
- Provide education and support to patients and families on disease management, treatment options, and lifestyle changes
- Collaborate with healthcare teams to ensure comprehensive and coordinated care
- Maintain accurate records and documentation of patient interactions and interventions
Required Skills and Qualifications:
- Master's degree in Social Work (MSW) from an accredited program
- Temporary state license or equivalent as a Registered Social Worker Clinical Intern, Associated Social Work license, or Provisional LCSW License (as required by state)
- Ability to work independently without supervision within two years or as per state guidelines
Supervision:
- Must have state temporary license, Registered Social Worker Clinical Intern, Associated Social Work license, or Provisional LCSW License (if required by state)
- Obtains necessary state licensure to work independently without supervision within the first two years or as required by state guidelines
