Patient Services Coordinator

Fresenius Medical Care is seeking a skilled Clinical Social Worker - ESRD to join our team. The ideal candidate will possess excellent communication and interpersonal skills and be able to work effectively in a fast-paced environment.

Purpose and Scope:

• The Clinical Social Worker - ESRD will provide psychosocial services to patients treated by the facility, including in-center and home dialysis patients.
• The Clinical Social Worker - ESRD will work with the healthcare team to promote positive adjustment, rehabilitation, and improved quality of life for our patients.
• The Clinical Social Worker - ESRD will collaborate with the interdisciplinary team to inform, educate, and support staff in understanding the emotional, psychological, and behavioral impact of Chronic Kidney Disease on the patient and family.

Responsibilities:

• The Clinical Social Worker - ESRD will assess patients' psychosocial status, strengths, and areas of need that may affect rehabilitation and optimal treatment outcomes as part of the comprehensive patient assessment.
• The Clinical Social Worker - ESRD will participate in care planning to identify effective interventions that will help the patient meet rehabilitation, treatment goals, and improve quality of life.
• The Clinical Social Worker - ESRD will provide monitoring and interventions for the patient to adjust to dialysis and achieve optimal psychosocial status and quality of life.

Requirements:

• A Master's degree in Social Work (MSW) is required.
• The Clinical Social Worker - ESRD must have state required licensure and meet the applicable scope of practice board and licensure requirements in effect in the State in which they are employed.
• No prior experience is required, but 0-2 years' related experience is preferred.
