About the Opportunity
This is a fantastic chance for a Pyschosocial Support Professional to take on a new challenge and make a meaningful contribution to the lives of our patients. Your key responsibilities will include providing information and assisting patients with referral to community resources, such as home health services and vocational rehabilitation, to facilitate optimal treatment outcomes.
You will also maintain current knowledge regarding local vocational/educational rehabilitation programs and assist patients with referral and access to these services to enable them to remain employed, become employed, or receive education.
