Responsibilities:
- Participate in care planning with patients and healthcare teams to identify effective interventions for rehabilitation and treatment goals.
- Utilize established social work theory and methods, as well as quality of life measurement instruments, to address barriers and meet patient treatment goals.
- Provide monitoring and interventions for patients to adjust to dialysis and achieve optimal psychosocial status and quality of life.
- Offer supportive counseling services to patients within the scope of clinical training and state licensure.
Requirements:
- Masters degree in Social Work (MSW) from an accredited program.
- Licensure to practice as a Social Worker in the state where the facility is located.
- Knowledgeable about local vocational/educational rehabilitation programs and able to assist patients with referral and access.
