What are the responsibilities and job description for the Social Worker - Kidney Disease Specialist position at Fresenius Medical Care?
As a social worker at Fresenius Medical Care, you will be working with patients who have kidney disease, providing them with psychosocial support to help them adjust to their treatment plan. You will work closely with the interdisciplinary team to ensure that our patients receive comprehensive quality care.
Main Responsibilities:
- Patient Assessment and Care Planning: You will assess patients' psychosocial status, strengths, and areas of need that may affect their rehabilitation and optimal treatment outcomes. You will then participate in care planning with the patient and healthcare team to identify effective interventions to help the patient meet their goals.
- Quality Improvement: You will provide psychosocial support and interventions to address non-adherence, quality outcome, and quality of life concerns for all patients based on acuity level. You will also participate in monthly Quality review meetings with the interdisciplinary team.
- Patient Education: You will assess patient knowledge of kidney disease for barriers that may affect adherence to treatment. You will work with the patient, family, and healthcare team to provide education tailored to the patient's learning style, communication barriers, and needs.
- Staff Training and Support: You will assist with interview process and decision to hire new personnel if requested by SW Manager/Senior Manager. You will also work with administrative support staff to maintain updated patient resource lists.
