Key Responsibilities:
- Assess patients' psychosocial status and strengths/areas of need that may affect rehabilitation and optimal treatment outcomes
- Collaborate with patients and healthcare teams to identify effective interventions that promote rehabilitation, treatment goals, and improved quality of life
- Provide monitoring and interventions for patients to adjust to dialysis and achieve optimal psychosocial status and quality of life
- Assess patient knowledge of kidney disease and provide supportive counseling services as needed
- Maintain current knowledge regarding local vocational/educational rehabilitation programs and assist patients with referrals and access to these resources
