Job Description
The Clinical Psychosocial Specialist will work as part of our multidisciplinary team to assess and address the psychosocial needs of our patients. This role requires strong communication and interpersonal skills, as well as the ability to work effectively with patients, families, and healthcare teams.
Key Responsibilities:
- Assess patient psychosocial status and develop individualized care plans to promote optimal treatment outcomes.
- Collaborate with patients and healthcare teams to identify effective interventions and strategies for addressing barriers to care.
- Provide education and support to patients and their families on kidney disease management, self-care, and healthy lifestyle choices.
- Maintain accurate records and reports on patient progress and outcomes.
Requirements:
- Master's degree in Social Work (MSW) or related field.
- State licensure required.
- 0-2 years' experience in clinical social work or related field.
