Working closely with the healthcare team, you will assess patients' psychosocial status, strengths, and areas of need that may affect treatment outcomes. Your expertise will help identify effective interventions to address barriers and meet patient treatment goals.
You will provide supportive counseling services to patients, as permitted within the scope of your clinical training and state license. Additionally, you will facilitate referrals to community resources, such as home health services and vocational rehabilitation, to enable patients to remain employed or achieve their education goals.
