- Collaborate with the healthcare team to identify effective interventions and support patients in meeting rehabilitation and treatment goals.
- Utilize established social work theory and methods, as well as patient education programs, to address barriers and meet patient treatment objectives.
- Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life.
Key Responsibilities
- Assess patient knowledge of kidney disease and identify potential barriers to adherence.
- Provide supportive counseling services to patients within the scope of clinical training and state license.
- Maintain current knowledge of local vocational/educational rehabilitation programs and assist patients with referrals.
