Your Responsibilities
- Assess patients' psychosocial status and identify barriers to rehabilitation and optimal treatment outcomes
- Collaborate with patients and the healthcare team to develop effective interventions and care plans
- Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life
- Assess patient knowledge of kidney disease and provide supportive counseling services as needed
Requirements
- Masters in Social Work
- State required license
