Key Responsibilities:
- Acknowledge patient's psychosocial status, strength, and area of need which may impact on rehabilitation and optimal treatment outcome.
- Collaborate with patients and healthcare teams to develop effective intervention strategies that meet patient goals and improve quality of life.
- Apply established social work theory, methods, and interventions to support patient adjustments to dialysis and maximize outcomes.
- Provide ongoing education and support to patients and families regarding kidney disease management, advance directives, and other relevant topics.
- Collaborate with interdisciplinary teams to facilitate transplant referrals and optimize waitlist management.
Required Qualifications:
- Masters in Social Work degree
- State licensure required
