Job description
Fresh Start provides immediate outpatient and telehealth access to MAT (medication assisted treatment), family medicine, substance use disorders, counseling, Hepatitis, PReP and HIV treatment, peer support, case management, job offerings and vocational training services. Fresh Start Health accepts KY insurance plans including Commercial, Medicare, Medicaid and Medicaid HMO plans. We also accept OH Medicaid, Caresource and Molina plans and we do not charge additional program fees of any kind.

The Substance Use Disorder Treatment and Recovery Loan Repayment Program (STAR LRP) provides loan repayment for individuals working in a full-time substance use disorder (SUD) treatment program like ours.
Role and Responsibilities:
Provide comprehensive outpatient clinical care to patients of all ages with a wide range of conditions, diseases, and injuries to ensure their needs are met on an ongoing basis.
Perform physical examinations of patients to evaluate health and diagnose illnesses
Order and peform diagnostic tests, such as x-rays and laboratory tests
Determine, plan, and initiate appropriate treatment, and adjust treatment if necessary
Prescribe and dispense medications
Document medical information of patients and maintain electronic medical records
Educate, instruct, and counsel patients and families on presenting health problems, treatment plans, and home care, and encourage and promote preventative health care and positive health behaviors
Request consultation or referral with other health care providers, such as physicians and clinics
Actively maintain knowledge of current medical research and trends, and support research projects
Qualifications for Nurse Practitioner:
Graduate degree from an accredited nursing school, such as a Master of Science in nursing
Must have a current license to practice as a nurse practitioner in the state of practice
Must have the necessary specialty certification
2-4 years of clinical work experience in Family Medicine and MAT (Medication Assisted Treatment)
Must be compassionate and personable
Ability to communicate effectively with patients and other medical staff
Strong leadership skills, self-confidence, and ability to remain calm in stressful situations
Excellent analytical and critical thinking skills
Exceptional active listening skills

Responsibilities:
- Provide comprehensive primary care and Medication Assisted Treatment services to patients across the lifespan
- Perform physical examinations and assessments, diagnose and treat acute and chronic illnesses
- Develop and implement treatment plans, including prescribing medications and therapies
- Order and interpret diagnostic tests, such as laboratory work and imaging studies
- Provide health coaching and education to patients and their families
- Collaborate with other healthcare professionals to coordinate patient care
- Document patient encounters and maintain accurate medical records
Qualifications:
- Valid Advanced Practice Registered Nurse (APRN) license
- Master's degree in Nursing or related field
- Certification as a Nurse Practitioner or Clinical Nurse Specialist in the appropriate specialty area
- Experience in infant care, health coaching, acute pain management, and vital signs assessment
- Proficiency in using electronic medical record systems (e.g., Cerner)
- Knowledge of catheterization, DME (Durable Medical Equipment), sonography, home care, and assisted living practices
