Weekend Start of Care RN - Residential Home Health & Hospice
Location: Chicago, IL
Salary: $80,000 per Year | Full-Time
Overview:
We are seeking a compassionate and dedicated Weekend Start of Care RN to join our Residential Home Health & Hospice team in Chicago, IL. As a Start of Care RN, you will be responsible for delivering exceptional care and providing essential support to patients in their homes on weekends.
Key Responsibilities:
- Conduct comprehensive patient assessments during the start of care process.
- Develop and implement individualized care plans for patients.
- Administer nursing care and provide support for individuals in home health and hospice settings.
- Ensure all patient documentation is accurate and up to date in accordance with regulations.
- Collaborate with the healthcare team and provide guidance on patient care needs.
Required Qualifications:
- Associate Degree or higher in Nursing.
- Valid Certified Cardiac Rehabilitation Professional (CCRP) certification.
- Valid Registered Nurse (RN) license in the state of Illinois (IL).
- Valid Regular Driver’s License (DL).
- Valid certifications:
- American Heart Association Basic Life Support (AHA BLS)
- American Heart Association Advanced Cardiac Life Support (AHA ACLS)
Schedule:
- Weekend shifts required for the Start of Care process.
