- Monitor patients' vital signs and overall health status, documenting changes accurately.
- Educate patients and families on health management, medication administration, and lifestyle modifications.
- Coordinate with interdisciplinary teams to ensure continuity of care.
- Maintain accurate records using electronic health record systems such as Cerner.
- Provide support for acute care situations, including urgent care needs when necessary.
