What are the responsibilities and job description for the HRDI Clinical Supervisor ACT position at Friend Health?
A full-time Clinical Supervisor who, in addition to administrative duties, provides direct services at least 25% of the time. A Master’s Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC) responsible for managing all aspects of the ACT fidelity model which includes efficiently identifying and integrating or coordinating all services necessary for successful and continued community placements for chronically mentally ill adults. This person will also be responsible for supervising a team of QMHP/MHP/CRSS/RN who will be responsible for the facilitation of direct services to clients referred to the program, including the provision of assessments, service and risk management plans, the provision of offsite individual, group, and family therapy and the provision of assistance with placement of clients within the community.
JOB DUTIES:
- Provides clinical leadership for ACT program. As a member of the treatment team of the program, is responsible for providing a broad range of treatment and case management functions for the population served. Provides clinical program coordination.
- Collaborates with external consultants and internal clinical and development staff to ensure fidelity to the
- ACT models
- Responsible for monitoring the program’s faithfulness to the ACT models and overseeing various other quality control and financial responsibilities.
- Clinical responsibilities including monitoring consumers’ status, delivering direct services to consumers, and supervising the clinical performance of team members including providing feedback to team members.
- Along with team members, assesses client needs, including documenting high-risk indicators in a manner
- that is clinically pertinent to the individual client.
- Facilitates, ensures compliances, and leads weekly (four times per week) interdisciplinary treatment team
- meetings with ACT team members including the ACT psychiatrist.
- Provides direct clinical assessment and intervention services to selected individuals, groups, and families.
QUALIFICATIONS:
Master's Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC)
5 years of relevant experience
