Position Summary
Position Summary
Renders routine nursing care to Members in accordance with established policies and procedures; performs related activities as assigned to complement the work of the nursing team. Support Culture of Excellence through active role with assigned neighborhood initiatives. Reports to Team Leader, Nurse in Charge (RN and/or LPN).
Essential Job Functions
- Performs care according to Member Care Cards.
- Gives daily hygiene care to Members including baths, oral hygiene, hair care, nail care, shaving, foot care; dresses and/or assists Members to dress as directed on the
- Member Care Cards.
- Makes beds; keeps Members’ rooms and belongings in order; keeps equipment neat and clean.
- Serves meal trays and assists with feedings as necessary.
- Passes nourishment’s to Members designated to receive them and records amount taken.
- Assists Members to and from bathrooms, on commodes or bedpans; renders care to incontinent Members and provides peri-care.
- Assists Members with range of motion, sitting, standing, and walking to maintain mobility. Applies supportive measures to help prevent deformity.
- Provides arm assist, cane assist, standby assist, walker assist, and wheelchair assist as directed on Member Care Card.
- Assists Members using lift belt, mechanical lifts, one and two person transfers as directed on Member Care Cards.
- Responsible for following care plans and skin care protocol on all Members.
- Performs procedures as outlined in manual such as: admitting, discharge, transfer of Members; specimen collection; taking and recording T.P.R. and BP; measuring and recording intake and output.
- Demonstrates excellent customer service skills.
- Transports Members to activities and/or appointments.
- Reports observations of Members/Patients to team Leader or Supervisor/Manager.
- Responsible for daily documentation in PCC (Point Care Click), adheres to all HIPPA regulations.
- May assist with Member activities as assigned.
Other Job Functions
- Accountable for daily completion of required daily documentation.
- Annually participates in 12 hours of In-service Education and Safety Programs of the Home; may attend selected meetings and workshops to improve job skills.
Qualifications
- A combination of experience, education, and/or training which substantially demonstrates the following knowledge and skills:
- High School diploma or GED
- New York State Certified Nursing Assistant certification required
- Good interpersonal skills and ability to communicate, and work well with both individuals and in groups.
- Willing and able to work as a team member.
Job Requirements
- Must follow Member Care Cards when performing care on Members.
- Must be able to work as a team Member with all staff on neighborhood and in Home.
- Requires ability to relate and be respectful to a wide variety of people and provide superior customer service to all including employees, Members, co-workers, vendors, family members and visitors.
- Must wear employee badge at all times while on-the-job.
- Required to follow Nursing Department dress code.
- Must report to work on time and adhere to attendance policy.
- Must be able to accept weekend assignment on a regular scheduled basis; may be assigned to evening or night shifts and holidays.
- Must remain current with in-service hours required.
- Must understand OSHA Bloodborne Pathogen Level 3 position as indicated in the Exposure Control Plan. Refer to the Exposure Control Plan and to Administration Policy #39 for further details.
- Must be able to maintain a high level of confidentiality regarding information about employees, Members, co-workers, vendors, family members and visitors.
- Must have reliable transportation.
Expectation of the Position
- Dependable, report to work on-time for every schedule shift (ready to work at beginning – start of shift) and work full schedule shifts unless physically unable or pre-approved Paid Time Off (PTO) which includes personal and vacation.
- Willing and able to work overtime and weekends as required by the position and/or approved by management.
- Have reliable transportation to and from work.
- Must be clean and neat in appearance and have a “friendly” service-oriented, cooperative, demeanor and supports Culture of Excellence initiatives by interactions with co-workers and/or Members.
- Abide by work-related / job procedures and organization policies; (refer to the Employee Handbook).

- Work is performed in a clean, well lit, “Home-Like”, nursing Home setting.

- May be requested to assist in the evacuation of Members or perform other procedures to protect the safety of Members, visitors and team members during emergency situations.
