Friends and Companions Adult Day Health Care is accepting applications for a Case Manager (LPN) to enhance the quality of participant management, maximize satisfaction and promote cost-effectiveness. The Case Manager shall be accountable for providing case management to the participants of the Home and Community Based Service program per 907 KAR 7:010 Section 8.
The Case Manager position is responsible for working with participants and their families to develop a Person-Centered Service Plan (PCP) to address identified needs, ensuring services address person-centered outcomes, and are provided in the participant’s environment of their choosing.
QUALIFICATIONS:
· A Licensed Practical Nurse (LPN) who has at least two (2) years experience as a professional nurse in the field of aging or disabilities.
· Maintains current American Red Cross or American Heart Association - CPR and First Aid Certification.
· Must be able to travel and work in varied home environments in all seasons of the year.
· Must have a current driver’s license and the ability and willingness to travel and reliable transportation.
· Position requires the physical ability to perform the job functions outlined in the Case Manager job description.
