The range listed is an estimate as it looks at the total compensation package as well as factors such as skill sets, training, education and organizational needs. SUMMARY :
The Friends For All Corporation TBRA Home Case Manager is responsible for providing comprehensive case management services to clients enrolled in the Tenant-Based Rental Assistance program. The role involves assessing client needs, developing individualized housing plans, coordinating supportive services, and ensuring compliance with program regulations to promote housing stability and self-sufficiency.
ESSENTIAL DUTIES AND RESPONSIBILITIES :
Conduct initial assessments to determine client eligibility for the TBRA program.Gather and document client information, including income, housing history, and personal circumstances.Develop and maintain comprehensive client files with all required documentation.Collaborate with clients to develop personalized housing plans that outline goals, resources, and timelines.Regularly review and update housing plans to reflect clients' progress and changing needs.Provide guidance and support to help clients achieve their housing stability and self-sufficiency goals.Identify and connect clients with necessary supportive services, such as employment assistance, financial counseling, healthcare, and education.Coordinate with external service providers to ensure clients receive appropriate and timely assistance.Monitor clients' participation in supportive services and adjust plans as needed.Assist clients in searching for suitable housing and navigating the rental process.Facilitate communication between tenants and landlords to address concerns and resolve conflicts.Ensure clients understand their rights and responsibilities as tenants.Maintain accurate and up-to-date records of client interactions, progress, and outcomes.Ensure compliance with all TBRA program regulations and reporting requirements.Prepare and submit regular reports on program activities and client outcomes.Provide ongoing case management to support clients in maintaining stable housing.Conduct regular follow-up visits to assess clients' housing situations and provide additional support as needed.Develop and implement strategies to address barriers to housing stability.Represent the TBRA program at community meetings, events, and forums.Advocate for clients' needs and promote program services within the community.Build and maintain partnerships with local service providers, housing agencies, and other stakeholders.Complete proper, accurate and timely documentation of services in writing and in Athena, CAREware, Evaluation Web, Redcap, Apricot, HMIS and any other database, as required.ADDITIONAL DUTIES AND RESPONSIBILITIES :
