JOB SUMMARY :
Friends of Children with Special Needs (FCSN) is a nonprofit organization. FCSN Coordinated Family Support (CFS) is seeking an energetic, patient, detail-oriented, and reliable direct support staff. Under direction from the Case Manager, the direct support professional provides services to adults with developmental disabilities who live in their own homes and out in the communities.
We are looking for people who enjoy working with people with developmental disabilities who may have complex service needs. This may include working with clients who have low verbal abilities, poor comprehension, and some behavioral challenges. We are also looking for people who are interested in teaching and role-modeling a positive lifestyle. The candidate's availability and flexibility to work when needed is a plus. In-Home Supportive Services (IHSS) providers are encouraged to apply. RBT certified is a plus.
JOB DUTIES : To empower individuals to live the most independent life desired, you will :
- Implements independent living skill curriculum in the areas of healthy and balanced meals, hygiene, sports, effective communication, community or neighborhood outings, shopping, money management, household maintenance chores, optimal health, social / leisure activities, self-advocacy, mobility training, etc.
- Provides close supervision and training to participants
- Provide direct care services, e.g. personal care aides, home health aides, etc. to clients as needed
- Provide transportation services to clients as needed
- Engaging and motivating participants in learning, practicing, and progressing
- Maintain a safe and clean living environment for participants. Minimizes health, safety, and any other risks to the well-being of participants
- Maintains a supportive and professional relationship with participants and their families.
- Documents participants' activities, medication intake, and progress daily.
- Submits complete and accurate participant updates to the Case Manager in a timely manner
- Administrative support, good verbal and written communication skills, great interpersonal skills, filing, enter and maintain client & prospect data in automated system.
- Be compliant with HIPPA regulations
- Performs first aid and cardiopulmonary resuscitation (CPR), and other emergency services (e.g. evacuation during a fire) as appropriate to the needs of participants
- Prepares special incident and adult dependent abuse reports as necessary
- Communicate effectively and proactively with the Case Manager concerning participant issues including situations when a participant becomes a challenge to self or others, there is a sudden illness or their non-aversive behavioral plan cannot be implemented as agreed upon
- Support clients in a person-centered manner
- Attend required training and staff meetings
- Adhere to all FCSN policies, procedures, and program rules
- Other duties as assigned;
