What are the responsibilities and job description for the Case Manager, Supportive Housing - HYS-HSS position at Friends of Youth?
The Case Manager of Supportive Housing provides comprehensive case management, counseling, and career guidance services to single, pregnant and/or parenting youth and their young children.
Reports To: Manager of Housing and Supportive Services
Schedule: Tuesday through Saturday 9:00AM to 9:30PM
essential duties
Program Delivery
- Provide ongoing career guidance and educational counseling.
- Provide education and role modeling to parenting youth.
- Identify education resources and job training services.
- Offer a series of workshops on interview preparation, career exploration, etc.
- Assist with locating financial aid resources.
- Assist with gaining access to ongoing healthcare.
- Assist with obtaining access to transportation for attention appointments, job interviews, school, etc.
- Provide case management in areas of education, employment, and personal and health needs.
Case Management Tasks
- Conduct client intake and assessments.
- Assist in the development of individualized housing plans.
- Provide weekly individual case management sessions with clients.
- Maintain accurate and updated participant records and data on progress towards goals.
- Complete quarterly program reports.
- Provide follow-up and aftercare services for clients who have left the service.
- Participate in agency and professional group seminars and conferences to increase professional knowledge.
Advocacy
- Develop referral network of specialized service providers.
- Collaborate with other case managers, local government, school, and area employers to create jobs and mentoring opportunities for clients.
- Work with community volunteers to develop new program resources and career options for clients.
- Advocate on behalf of youth and families for system change.
job requirements
a. Education and Experience
- Bachelor’s degree in a human-service field preferred.
- Minimum of 2 years of relevant working experience, including supporting youth, young adults and families experiencing homelessness.
- Experience providing case management services.
- Experience with motivational interviewing and employment coaching preferred.
b. Additional Requirements
- Must satisfactorily pass criminal history check.
- Must satisfactorily pass tuberculosis (TB) test.
- Must have reliable transportation and a valid Washington State driver's license (or be able to obtain one by date of employment).
- Employees and volunteers who operate their own or Friends of Youth vehicles on Friends of Youth business, must have a safe driving record as defined by Friends of Youth.
- Employees and volunteers who operate their own vehicle(s) on Friends of Youth business must carry auto liability insurance that complies with the requirements of Friends of Youth.
- Must successfully complete the following health certification requirements within 30 days of employment: CPR, First Aid and Bloodborne Pathogens.
- Proof of COVID-19 vaccination is required. There are no exemptions.
