The Case Manager of Supportive Housing provides comprehensive case management, counseling, and career guidance services to single, pregnant and/or parenting youth and their young children.

Reports To: Manager of Housing and Supportive Services 

Schedule: Tuesday through Saturday 9:00AM to 9:30PM

essential duties

Program Delivery

  1. Provide ongoing career guidance and educational counseling.
  2. Provide education and role modeling to parenting youth.
  3. Identify education resources and job training services.
  4. Offer a series of workshops on interview preparation, career exploration, etc.
  5. Assist with locating financial aid resources.
  6. Assist with gaining access to ongoing healthcare.
  7. Assist with obtaining access to transportation for attention appointments, job interviews, school, etc.
  8. Provide case management in areas of education, employment, and personal and health needs.

Case Management Tasks

  1. Conduct client intake and assessments.
  2. Assist in the development of individualized housing plans.
  3. Provide weekly individual case management sessions with clients.
  4. Maintain accurate and updated participant records and data on progress towards goals.
  5. Complete quarterly program reports.
  6. Provide follow-up and aftercare services for clients who have left the service.
  7. Participate in agency and professional group seminars and conferences to increase professional knowledge.

Advocacy

  1. Develop referral network of specialized service providers.
  2. Collaborate with other case managers, local government, school, and area employers to create jobs and mentoring opportunities for clients.
  3. Work with community volunteers to develop new program resources and career options for clients.
  4. Advocate on behalf of youth and families for system change.

job requirements 

a. Education and Experience

  1. Bachelor’s degree in a human-service field preferred.
  2. Minimum of 2 years of relevant working experience, including supporting youth, young adults and families experiencing homelessness.
  3. Experience providing case management services.
  4. Experience with motivational interviewing and employment coaching preferred.

b. Additional Requirements

  1. Must satisfactorily pass criminal history check.
  2. Must satisfactorily pass tuberculosis (TB) test.
  3. Must have reliable transportation and a valid Washington State driver's license (or be able to obtain one by date of employment).
  4. Employees and volunteers who operate their own or Friends of Youth vehicles on Friends of Youth business, must have a safe driving record as defined by Friends of Youth.
  5. Employees and volunteers who operate their own vehicle(s) on Friends of Youth business must carry auto liability insurance that complies with the requirements of Friends of Youth.
  6. Must successfully complete the following health certification requirements within 30 days of employment: CPR, First Aid and Bloodborne Pathogens.
  7. Proof of COVID-19 vaccination is required. There are no exemptions.

  8. Able to establish and maintain positive working relationships with youth and young adults.
  9. Knowledge of trauma-informed care, harm reduction philosophy. and strengths-based practice.
  10. Able and willing to travel to multiple local sites and other locations in personal vehicle (mileage reimbursement provided).
  11. Proficient skill set with Microsoft Office programs.
