SUMMARY: Evaluates and provides services to children ages 0-â€'5 and their families are assigned counties, coordinates service, and provides one-on-oneâ€' instruction. Develops initial plans of care for all eligible children. Establishes and maintains good working relations with all referral sources.

EDUCATION And/or EXPERIENCE
Bachelor's degree (B. A.) in Early Childhood Education/Special Education, or ALP in Early Childhood/Special Education, or four-year degree in related area with some experience in working with children and families or meet the minimum requirements for Developmental Therapist as set forth by First Connections standards.
Language Skills
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
Mathematical Skills
Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry.
Reasoning Ability
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

- Responds to all referrals according to procedures.
- Conducts initial evaluations, and evaluation programming conferences in a manner that encourages participation by all members.
- Ensures Part C due process is maintained for all clients accordingly.
- Maintains Service Coordinator Certification through First Connections and attends all First Connections required training.
- Acts as Service Coordinator for conferences when indicated.
- Enters data into CDS and Credible according to procedures.
- Monitors each child's progress on an on-going basis and makes modifications/changes to facilitate learning.
- Coordinates client therapy program with speech, physical, and occupational therapies.
- Monitors/maintains minimum 80% daily productivity and makes needed adjustments to caseload as indicated. Completes and provides weekly Productivity Report to Supervisor.
- Participates in staff training as assigned by Administration, including out-of-area travel.
- Performs Early Intervention services throughout the service area as needed in compliance with First Connections.
- Actively participates in various marketing events throughout the community and within facility on a routine basis.
- This is a safety-sensitive position.

- The Code of Conduct and Compliance Investigation Procedures as read and discussed in the new employee training must be followed at all times.
- Consumers' progress notes be maintained and completed daily or as needed on the prescribed forms.
- Consumers' records must be maintained weekly, monthly, quarterly, and yearly. All information must be filed in consumers' records as needed and updated as prescribed by funding sources.
