SUMMARY Under the direction of the Executive Director/ Director of Health Services is responsible for the coordinating supervising and directing the work of the Memory Care/Summerhouse staff and ensures the resident needs are met according to Federal and State regulations as well as Company Policy.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Provides quality Customer Service efficiently to residents, families, co-workers and vendors in a manner to ensure satisfaction.
Supervises all activities of Memory Care to assure quality care.
Supervises all memory care staff to include; hiring, terminating, couching, training, scheduling and directing.
Ensures 24-hour provision of nursing services within Memory Care.
Coordinate and perform resident assessment and plan of care evaluations to ensure residents are at the appropriate level of care and services. Consult with DON for professional assistance.
Complete rounds as necessary, responds to medical alert calls as required.
Develops and implements a memory care program specifically to meet the needs of the community residents.
Conducts initial and ongoing State required staff training for residents with Dementia.
Conducts cognitive assessments of residents for the initial admission to Memory Care, as required by the State on an ongoing basis.
Orients new staff to ensure quality of care for residents, dispenses work assignments and assures completion before end of shift.
Keeps proper resident care documentation and ensures charts are in order, maintains required documentation as per Federal, State and Company Policy.
Acts as liaison between family, residents, and physicians. Supervises the health care services and programs to residents.
Organizes support groups for families and community residents
Coordinates resident admissions and transfers with other departments and/or community agencies.
Supervises and participates in resident interdisciplinary care plan conferences. Supervises documentation in resident health records and reviews scheduled audits.
Participates in budget planning and monitoring of fee scheduling and charging for services and cost effective utilization of resources
Responsible for orientation and on-going in-service education of all assisted living nursing staff.
Promotes stimulation, comfortable, uplifting environments for residents, families and staff.
QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION AND/OR EXPERIENCE: Experience in long-term care community health, geriatric and/or rehabilitative nursing desirable. 5 years’ experience working in a senior community and have supervisory experience. Preferred LVN licensure by the California Board of Nursing.
