SUMMARY: Provides nursing care including assessments to the residents of the residential care facility for the elderly. Assists the Administrator and Lead Coordinator in planning, coordinating, monitoring and implementing nursing care as needed. Assists with coordination of services with other agencies as needed. Schedule: Thursday - Sunday.

Resident Care
- Assist in assessment of potential residents for placement at our facility.
- Upon admission of new residents and as needed, will develop and implement nursing care plans.
- Provide professional client-centered nursing care that is consistent with the Nursing and Program philosophy, policies and procedures, and within scope of practice.
- With client involvement, will assess, plan, and evaluate the nursing care delivered.
- Coordinates nursing care with the interdisciplinary team.
- Will assess, develop, and implement a client-centered care plan, and maintain ongoing evaluation of service plan with client participation.
- Provide residents with information related to interventions and activities as indicated.
- Direct and supervise care given by other unlicensed nursing personnel.
- Maintain confidentiality and clients’ rights.
- Provide client teaching and document as necessary.
- Provide input in the formulation and evaluation of standards of care.
- Provides direct client care as necessary, including assessing vital signs and assisting clients to complete daily hygiene, grooming, and housekeeping tasks (ADL’s).
- Provide leadership and supervision to direct care staff as needed.
- Makes and/or reviews daily assignments and adjusts as necessary. Assures all assignments / duties are covered and completed.
- Attend orientation and in-service training as required.
- Gives direction and on the spot training, as necessary.
- Attends and participates in team planning and other client-centered meetings as assigned. Reports on clients’ needs and progress.
- Participates in team decisions to individualize care of residents.
- Assures clients receive the prescribed diet. Assists in the dining room at meals, and reports discrepancies to the Supervisor. Reviews meal refusals according to current policies and procedures.
- Be economical and careful with facility property and supplies.
- Other duties as assigned by Administrator.
FOLLOW FACILITY PROCEDURES FOR:
Resident Care
- Administration of medications and treatments and be knowledgeable of all medications administered, including side effects.

- Physician’s orders, including transcription.
- Legible and pertinent documentation including the Weekly Nursing Summary.
- Notify physician, conservator / responsible party of admission and all significant changes in client status.
- Notify Supervisor of any unusual events.
- Participate in staff development by attending and conducting in-service education.
- Identifies educational needs, and serves as a preceptor for new staff and students.
- Completes orientation classes, Non-violent Crisis Intervention training, and attends monthly licensed staff meetings.
Direct Services and Case Management
- Provides or arranges counseling, assistance with daily living skills, social skills, grooming and personal hygiene, housekeeping, meal preparation, shopping, money management and managing disabilities and/or psychiatric symptoms.
- Assists in identifying and accessing medical, educational, social, prevocational, rehabilitative or other community services for consumers.
- Provides direct services for individual and group learning experiences to assist the consumer to achieve his or her objectives.
- Participates in the assessment, planning, implementing and evaluations of the individual’s goals in conjunction with other staff, case managers, families and/or other local providers, as appropriate.
- Assesses, plans, implements and evaluates the program effectiveness of achieving the resident’s objectives. Assists in identifying barriers to these achievements and plans to overcome them.
- Appropriately manages any dangerous consumer behaviors or dangerous conditions observed.
Qualifications
Required Qualifications:
- Must be able to pass Department of Justice criminal record clearance.
- Willing and able to problem-solve and communicate well with staff and residents.
- Must demonstrate cultural competency and good reasoning skills.
- Must have an active Nursing License.
