Description We are seeking a Nurse LPN or RN to oversee the health and wellness of our residents, in coordination with our Executive Director's. This part time position would be split between two of our locations in Kenosha (Azalea) and Racine ( Willowgreen) We care about your work-life balance and equip you with the tools you need to be successful at work. The Community Nurse assumes primary responsibility for coordinating the health care services on a 24 hour / 7-day week basis of assigned resident. Participates in individual service planning as well as renewal and maintenance of these plans. Recommends health care service goals and assists in their implementation. Oversees and coordinates work of House Managers and care givers to meet resident needs. Provides clinical services as needed to residents and trains staff to meet resident needs. Coordinates delegation of medication administration / assistance and health care tasks per state regulations, Nurse Practice Act.
