What are the responsibilities and job description for the Clinician - Discharge Planner - Inpatient Units - 2 shifts - Weekday and Weekend Day position at Frontier Behavioral Health?
SHIFTS:
- Monday -Friday Day Shift - Full Time
- Saturday/Sunday 7AM-7:30PM (PART-TIME - .6FTE) - There is added shift differential as outlined below.
LOCATIONS:
- Weekday Shift will be at Calispel Inpatient Unit
- Weekend Day will cover both Inpatient Units - Foothills & Calispel
The Discharge Planner will provide clinical services to patients admitted to Evaluation and Treatment (E&T) inpatient units. Coordinates and plans patient discharge, connecting clients to various resources such as housing and community services. Utilizes psychosocial principles in providing services to patients during their stay, facilitating groups and engaging in 1:1 counseling with patients as needed. Must have the clinical knowledge to assess in terms of suicidal (including past attempts/suicidal ideation), homicidally and ability to care for self; safety needs; orientation and thought process (formal or informal mental status exam); and patient’s affiliation with other therapeutic alliances; support networks.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Provide case management services which may include coordination of housing, Medicaid enrollment, insurance enrollment, coordination of court dates, appointments or connection with other community resources and follow up services.
- Provide 1:1 counseling service to clients as needed.
- Facilitate psychoeducational groups as a component in the therapeutic treatment plan.
- Provide outreach and follow up to individuals discharged from the Evaluation and
- Treatment Facilities, as appropriate.
- Coordinate with various mental health and chemical dependency treatment agencies, including DCR's, psychiatric and medical hospitals, and the criminal justice system.
- Complete a psycho-social intake with each client.
- Participate in daily multi-disciplinary team meetings.
- Participate, discharge planner group meetings in-FBH educational and administrative meetings as directed.
- Perform FBH Discharge Planning charting requirements, correspondence, Dayforce and other paperwork in compliance with FBH policies.
- Other duties as assigned.
QUALIFIED APPLICANTS WILL HAVE:
REQUIRED:
- Master’s degree in the behavioral sciences.
- Must submit for Agency Affiliation within 30 days of hire
- Active BLS within 90 days of hire and must maintain active BLS
- Must have reliable transportation, insurance and an active driver’s license and be willing to use personal vehicle in the course of work.
PREFERRED:
- At least one-year related experience working with psychiatrically impaired populations.

PAY RANGE (Hourly/ Non-Exempt):*
Discharge Planner - No License: $31.84 - $38.79
Discharge Planner - Licensed: $33.70 - $41.06
*All pay is based on longevity at Frontier Behavioral Health; thus the starting pay for this position is as follows:
Discharge Planner - No License: $31.84 per hour.
Discharge Planner - Licensed: $33.70 per hour.
There is added shift differential of 10.98% to the pay above for the Saturday & Sunday 7AM-7:30PM shift only.
