The Program for Assertive Community Treatment (PACT) is an intensive, community-based multidisciplinary treatment team that provides person-centered care to a shared caseload of adults whose needs are not able to be met through traditional outpatient services. The Mental Health Care Provider (MHCP) provides clinical behavioral health services to clients experiencing thought disorder symptoms within their professional scope and collaborates with other care team members assigned to support client’s progress toward established goals and objectives. Services provided by the MHCP include but are not limited to engagement, supportive listening, psychoeducation, behavioral health counseling, care management, skill building, crisis assessment, prevention and intervention, resource education, and referral to internal and external resources with individuals in virtual, office, and community-based settings.
PACT operates from 8 a.m. to 5 p.m., 7 days a week. The PACT team is the first-line crisis evaluator and responder during hours of operation, and the MHCP must participate in responding to crisis intervention as needed within standard hours of operations. he PACT Team participates in rotating weekend and holiday shifts to provide coverage for our clients during non-business days. If a weekend day or holiday shift is worked by a PACT employee, the PACT employee will be able to take a different day off that same week.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
  • Provide episodic, trauma-informed, person-centered counseling utilizing evidence-based practices within scope of practice.
  • Maintain client records and all other required documentation in a timely and confidential manner in accordance with agency policies and procedures.
  • Conduct risk assessments, develop and implement safety plans, and provide crisis intervention and stabilization services as needed, including facilitating access to inpatient facilities for behavioral health care.
  • Facilitate and co-facilitate client treatment groups.
  • Represent the agency and its services to the community in a positive and professional manner.
  • Demonstrate teamwork and goal-oriented productivity.
  • Operate as a member of a multi-disciplinary team.
  • Support team in achieving direct service goals. Contribute to out-reached based, consumer-centered treatment, rehabilitation, and support. 75% of services must be provided on an outreach basis in the community.
  • Participate in daily multidisciplinary staff meetings and regular supervision.
  • Participate in inpatient discharge planning for PACT or potential PACT clients.
  • Participate in any required training, consultation calls, and onsite visits or reviews when required by contract.
  • Accept delegation and follow through with specific expanded assignments and responsibilities within FBH system such as quality improvement teams, task groups, committees, consultations, mentoring or training responsibilities.
  • Establish and maintain collaborative working relationships with; clinical, administrative and support staff, community partners, clients, and family/natural supports.
  • Identify and include the client’s support network and applicable involved community partners in care coordination.
  • Develop and implement care plans with measurable objectives and obtainable goals, in collaboration with the client, evaluating clients progress and adjusting as needed.
  • Help consumers become less socially isolated and more integrated into the community by encouraging participation in community activities.
  • Provide care management services to clients including:
  • Assist clients in identifying and accessing resources and collaborate with community resources on behalf of clients.
  • Assist clients in the development of healthy coping mechanisms in individual or group settings.
  • Provide employment and housing assistance as appropriate and necessary.
  • Offer psychoeducation on client diagnosis.
  • Comply with all state WAC requirements and FBH policies and procedures.
  • Perform FBH charting requirements, correspondence, Workday and other paperwork in a complete and timely manner.
  • May be assigned to supervise bachelors level interns.
  • Other duties as assigned
QUALIFIED CANDIDATES WILL HAVE:
REQUIRED:
  • Bachelor’s degree in social work, psychology, or a related field from an accredited college or university.
  • Must submit for Agency Affiliated Counselor Registration within 30 days of hire.
PREFERRED:
  • One year of experience providing mental health services in a behavioral health care setting.
  • Experience with crisis intervention.
KNOWLEDGE, SKILLS & ABILITIES:
  • Must have reliable transportation, insurance and an active drivers’ license and be willing to use personal vehicle in the course of work.
  • Ability to develop knowledge of Washington Administrative Codes and statutes pertaining to the treatment and delivery of mental health services.
  • Ability to communicate and engage effectively with individuals of varied backgrounds, trauma experiences, cultures, education level, and socio-economic status.
  • Demonstrated ability to apply active listening techniques conveying respect, professionalism, engagement skills, positive attitude, and empathy.
  • Must possess excellent communication skills, including the ability to send and receive oral and written messages, in a clear, positive, prompt, and appropriate manner, while maintaining customer confidentiality.
  • Ability to learn the PACT model and effectively deliver groups, skills training, brief/focused therapy, and community-based interventions for adults.

  • This position provides client services in-office and within the community at large on a occasional to frequent basis. Community settings include but are not limited to residences, group/congregate living settings, hospitals, shelters, the streets or anywhere in the community at large.
  • Some environments may not be ideal in terms of cleanliness or being habitable.
  • Occasional frequent travel between locations is required.
  • The ability to drive a vehicle, traverse distances up to several city blocks and on various terrains, navigate stairs, get in and out of vehicles frequently is required.
  • Daily use of a computer is required.
  • May be required to lift 10 - 20 pounds.
  • The following terms and conditions apply to new $2,000 Sign On Bonus:
  • Internal transfers into PACT are not eligible for the sign-on bonus, except for PRN employees who accept a full-time PACT position.
  • If you receive the $2,000 sign on bonus, you will not be eligible for an internal transfer out of PACT for one-year, absent extenuating circumstances and approval by the Chief Human Resources Officer.

PAY RANGE (Hourly/ Non-Exempt): $25.89-$31.53*
  • All pay is based on longevity at Frontier Behavioral Health; thus, the starting pay for this position is $25.89 per hour.
