SUMMARY: To provide supervision and oversight, plan, develop and aid in the implementation of the Supported Housing Program goals and objectives.. Position will build on existing community relationships as well as establishing additional community partners that will support the Housing First principles in assisting participants in finding permanent housing solutions.
EDUCATION AND EXPERIENCE:
Education: Bachelor’s degree in mental health related field preferred.
Licensure: Must possess valid driver’s license.
Certification: N/A
Experience: Experience working in the mental health field preferred.
Knowledge/Skills: Ability to maintain composure in difficult situations.
Verbal/written communication skills.
Possess interpersonal skills and flexibility to relate to changing situations.

MAJOR DUTIES AND RESPONSIBILITIES:

Inform supervisors of program’s progress toward goals, noting any potential and/or current staff and/or consumer issues.
Complete/monitor staff documentation in consumers’ records and the FH electronic health record (EHR). Accurately enter and document all required data in the EHR system and run required reports to review and correct any data entry errors.
Attend in-service trainings as needed.
Complete assessments to verify consumer eligibility for housing services and provide education, linkage, and referral to public and private agencies.
Partner with consumers to develop and implement Housing Stability Plan
Coordinate with consumer agencies to assist consumers in maintaining benefits.
Monitor Individual Service Plans (ISP), case assignments, preadmission screenings, staff outreach, training projects, progress notes, discharge summaries, and program productivity, making recommendations which would maximize fees and insure current accurate treatment plans completions.
Respond directly to housing-related crisis and coordinate with other emergency response systems to prevent and address another recipient crisis.
Link to transportation and/or provide transportation for consumers as needed.
Provide Outreach to landlords and Facilitates landlord-tenant communication and monitors basic lease compliance.
Coordinate and complete unit inspections before moving in and at least annually; ensure ongoing compliance with habitability and affordability standards.
Conduct in-home visits at least monthly to assess housing stability and progress toward supportive housing plan goals.
Assist in lease negotiations; assist with relocations and moves as necessary.
Coordinate minor facility maintenance and work with landlords to ensure landlords complete necessary repairs on a timely basis.
Maintain all obligations relating to certifications and expand self-knowledge and education through appropriate training opportunities.
Complete all requisite training in a timely manner as mandated by the agency and/or state, federal and local regulations.
Remains current in knowledge of community agencies and resources, both internally and externally, and how to access resources in order to provide information, referrals, and a seamless separation of care for individuals served by the
