Description
Under general supervision of the Young Adult & Rapid Rehousing department, and working collaboratively with other Youth Homelessness Demonstration Program agencies, provides case management services to homeless and formerly homeless young adults (18-24) utilizing the Transition in Place housing model to facilitate rapid exit from shelter, using evidenced based best practices such as Trauma Informed Care, Motivational Interviewing, Harm Reduction, and Housing First. Develops and implements the goals and objectives in the client’s Individualized Service Plan (ISP) to maintain housing and meet the individual’s identified goals, including but not limited to decrease in distress caused by metal health symptoms and substance use issues, increased income, improved interpersonal relationships, increased self-sufficiency, and increased income. Recertifies program eligibility, makes home-visits, and follows-up with participants to assist in maintaining their housing and independence. Supports a culture of diversity, equity, and inclusion by employing anti-racist practices and principles within the department and across the agency.
This is a non-exempt, union position with a minimum-starting wage of $20.00 per hour.
The hours of work are Monday through Friday, 8:30am – 5:00pm.
Qualifications
Qualified candidates must possess a Bachelor’s degree in a social work, counseling, or social service field. Lived experience of homelessness and/or housing instability and embodiment of the diverse identities of young adults who experience homelessness in Cuyahoga County they serve, particularly, but not limited to, BIPOC, LGBTQIA , and Young Adult. Experience providing services to the target population and/or prior case management experience preferred.
Valid Ohio driver’s license with less than six points and proof of automobile insurance, adhering to the minimum requirements of Ohio’s Financial Responsibility Act.
Salary : $20
