Description
Under general supervision of the Domestic Violence Rapid Rehousing department, provides case management services to homeless and formerly homeless individuals and families fleeing domestic violence or human trafficking using evidenced based best practices such as Trauma Informed Care, Motivational Interviewing, Harm Reduction, and Housing First. Develops and implements the goals and objectives in the client’s Individualized Service Plan (ISP) to maintain housing and meet the individual’s identified goals, including but not limited to decrease in distress caused by metal health symptoms and substance use issues, increased income, improved interpersonal relationships, increased self-sufficiency, and increased income. Recertifies program eligibility, makes home-visits, and follows-up with participants to assist in maintaining their housing and independence. This is a non-exempt, union position with a minimum-starting wage of $20.00 per hour. The hours of work are Monday through Friday, 8:30 – 5pm.
Qualifications
Qualified candidates must possess a Bachelor’s degree with specialization in social, behavioral or human services with a minimum of one year professionally supervised work in the delivery of mental health, chemical dependency and/or social services. Valid Ohio driver’s license with less than six points and proof of automobile insurance, adhering to the minimum requirements of Ohio’s Financial Responsibility Act.
