RESPONSIBILITIES:
- Conducts patient assessments and assists the physician in evaluating the level of function by applying diagnostic and prognostic functional ability tests.
- Guides the patient in the use of therapeutic, creative, and self-care activities to improve functioning.
- Observes, records, and reports to the physician the patient’s response to treatment and changes in the patient’s condition.
- Instructs patient, significant others, and staff in the areas of therapy in which they can participate to assist the patient.
- Visits patient according to Plan of Treatment; completes a progress note for each visit; and submits progress notes to the agency on at least a weekly basis.
- Participates in staff conferences and committees as necessary.
- Attends all education and in services as required.
- Reviews and countersigns all progress notes written by the licensed and certified COTA.
- Submits an initial evaluation and participates in developing the plan of treatment,
- Prepares a written discharge summary to the physician and a written summary report on patient’s condition at least every 60 day
- Supervised by: Director of Nurses/Alternate Director of Nurses
- Supervises: Other occupational therapist and assistants in their employment
- Graduate of an American Occupational Therapy Assoc. approved program
- Current license to practice occupational therapy in Texas
- Current driver’s license; with current automobile insurance.
