Responsibilities:
- Provides high-quality patient care, focusing on patient safety and quality of care.
- Collaborates with interdisciplinary teams to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.
- Documents patient care given, using clear and concise language.
- Monitors, records, and communicates patient condition as appropriate to care team, physician, patient, and family.
- Works as an advocate for the physical and emotional well-being of the patient.
