The Position
The Case Manager / Clinical Case Manager provides case management along with service and resource coordination with clients that are low-income and may be in crisis or emergency situations. Clients served include adults, seniors, and families. Case management focuses include housing stabilization and retention, access and engagement with physical and mental healthcare providers, and supporting seniors aging in place. The Case Manager / Clinical Case Manager will work collaboratively with clients, community-based providers, and public and private agencies to guide households in overcoming obstacles to self-sufficiency. Through advocacy, planning, coordinating, and linkage to community resources, the Case Manager / Clinical Case Manager will identify risks, promote strengths, and equip clients with the skills and knowledge needed to overcome sources of stress, strengthen resiliency, and health disparities. Services are provided in community settings, FSA offices, and in the homes of clients. This position reports to the Housing Authority Program Supervisor.
