What are the responsibilities and job description for the Health Home Care Manager (Bilingual/Spanish) position at FSS NY?
Please note that this job is remote and flexible but requires one monthly visit to each child in your caseload. For this position, you must complete trainings before you begin. Upon completion of trainings, you are assigned a Supervisor and given your first few per diem cases. You can then grow your caseload each month.
FSS is looking for Care Managers.
The main responsibilities of a Health Home care manager include:
- Coordinating the care and well-being of the families and members they serve by providing the necessary referrals and services
- Serving as a liaison and central point of contact in order to ensure that families’ needs are met, their choices are valued, and their personal preferences are included in their plan of care
- Monitoring services to ensure that healthcare goals are being achieved in an effective and timely manner
- Ensuring that care management services are individualized, family-centered, and community-based
- Managing administrative and billing needs as applicable
- Promote and maintain professional standards
You would perform activities like:
- Monitor and assist with medical services and needs;
- Locate community resources and inform families about community-sponsored events;
- Coordinate educational needs and resources;
- Locate support groups for individuals and families;
- Assist with rehab and transitional services;
- Monitor Home and Community Based Services (HCBS), such as respite services, prevocational training, and ComHab services;
- Provide referrals for clinical therapeutic services;
- Assist with social services such as food benefits, home and health benefits, transportation, and community-based programs.
Required Qualifications:
· At least a Bachelor's Degree
· NY State ID
· Experience with MS Word, Excel, and Google
Preferred Qualifications:
· Experience working within the healthcare industry and with health care insurance
· Experience working in a hospital, physician’s office, or medical clinic setting
· Experience working in a call center
· A clerical or administrative support background
· Knowledge of ICD-9 and CPT codes
· Fluent in Spanish
