What are the responsibilities and job description for the Mobile Case Manager position at Full Circle Recovery Services?
The Mobile Case Manager will be a mobile position to assist clients with crisis intervention and completing tasks as needs arise. The position will encompass a variety of roles including, but not limited to assisting case management, scheduling, and keeping appointments as well as completing all notes within 24 hours of appointments being made, and other related tasks, duties, and responsibilities as assigned.
Mobile Case Manager, under the supervision of the Mobile Case Manager Lead, duties include:
- Assess clients’ physical and mental wellness, needs, preferences, and abilities and use these to develop tailored care plans with treatment team
- Working to link clients with support networks and other health care professionals within the agency and externally.
- Listening to clients’ concerns and providing appropriate intervention as required.
- Attendance of 1 hour per week of clinical supervision as requested by the Ohio Chemical Dependency Professional Board to maintain current license/certification.
- Recording clients’ progress including charting referrals, home visits, and other notable interactions via agency EMR/EHR
- Ensuring development and maintenance of transitional client assignments, placements, vocalized therapeutic concerns, and needed services.
- Evaluating clients’ progress periodically, making and communicating needed adjustments with staff as needed to improve outcomes for clients.
- Provide case management services in their guest housing primarily focused on daily living skills, living in a communal environment, ensuring basic housing needs are met, and addressing client concerns that arise at their sober living.
Requirements:
- Minimum High School Diploma
- Experience in substance use disorder field preferred
- Must have a CDCA or higher
- Carry out detailed written and/or oral instructions, along with the ability to understand human behavior
- Must maintain a professional demeanor while dealing with emotionally charged issues and individuals
- Effectively present information and respond to questions from a diverse demographic group
- Ability to draft reports and correspondence
- Effective Communication
- Active listening skills
- Demonstrate organization, leadership, and team building
- Always maintain confidentiality displaying a professional disposition towards fellow staff.
- Maintain a positive working relationship with CSL staff and consumers alike.
- Must have valid driver's license
