What are the responsibilities and job description for the Re-Entry Case Manager/CDCA position at Full Circle Recovery Services?
The successful candidate will monitor and provide direct clinical care and intensive case management to the clients of Clinic Security and Logistics, Inc. The successful candidate will collaborate with the appropriate providers to integrate substance use disorders care for the clients with an understanding of the additional barriers and needs related to involvement with the criminal justice system. The successful candidate must triage care for clients and consult with the appropriate staff as necessary and within their licensure standards to ensure the highest degree of quality care is always maintained for all clients.
Primary Duties:
- Complete assigned case management schedule visits in-person or via telehealth if approved by Management.
- Capture expected clinical service hours.
- Efficiently complete required documentation for billable and non-billable services.
- Comply with Company policies and procedures.
- Assist clients in adhering to treatment plans.
- Counsel clients in individual and group sessions to assist them in dealing with substance abuse.
- Assist clients in understanding, problem solving and navigating barriers to independence and long-term recovery through education, advocacy and linkage to resources.
- Educate clients and community members about mental and physical illness, abuse, medication, and available community resources.
- Monitor, evaluate, and record client progress concerning treatment goals.
- Refer client or family to community resources for housing or treatment to assist in recovery from mental or physical illness, following through to ensure service efficacy.
- Attend and participate in continuing education, online training, and staff meetings.
- Educate clients by providing direction and guidance in a professional manner.
- Model pro-social behaviors and effective interpersonal interactions.
- Cultivate effective and respectful rapport with clients and staff.
- Maintain confidentiality of client’s pertinent health information in accordance with HIPPA guidelines.
- Perform other duties as requested within the CDCA licensure standards.
Qualifications and Education Requirements:
- Must possess a current, unencumbered, active license to practice as an CDCA in the State of Ohio.
- Experience working with the substance abuse population preferred.
- Experience working with clients with criminal justice system involvement preferred.
- Must carry out detailed written and oral instructions, along with the ability to understand human behavior.
- Must maintain a professional demeanor while dealing with emotionally charged individuals and issues.
- Must effectively present information and respond to questions from a diverse demographic group.
- Must have the ability to work harmoniously with others.
- Must be able to make decisions at times with imperfect information.
- Must have the ability to write reports and correspondence.
- Must have effective listening and communication skills.
- Must be knowledgeable of clinical practices and procedures, as well as laws, regulations, and guidelines that pertain to Mental Health and Addiction Services.
- Must be able to cope with the mental and emotional stress of the position.
- Must be able to lift, push, pull or move up to 50lbs.
WORKING CONDITIONS:
- The Medical Marijuana Registration Card is not accepted for any employee.
- Work in an office setting, as well as the medical service area.
- Move intermittently during working hours.
- Subject to frequent interruptions.
- Involved with clients, personnel, visitors, etc., under different conditions and circumstances.
- Subject to possible hostile and emotionally charged individuals and issues.
