POSITION SUMMARY
This position requires management and assistance for patients of Williamson Medical Center, providing for short-term counseling/crisis management for patients and their families, who are impacted by adjustment to illness or diagnosis, change in residence or lifestyle, and/or financial concerns which are a result of their illness and hospitalization. This counseling/crisis management includes assistance with psycho-social, alcohol and drug problem assessment and management, discharge planning with appropriate referrals, education for patients/families and the appropriate use of resources. The Social Worker actively participates in consistent collaboration with physicians, staff, patients, families, and community agencies in the discharge planning process. This position requires that early discharge planning occurs, communicating effectively and the appropriate development of a discharge plan; therefore, alleviating/reducing anxiety for patients and their families. This role works within the knowledge and regulations related to CMS, Commercial Payers and Bundle programs in delivering appropriate discharge planning.
POSITION REQUIREMENT
Formal Education / Training:
Requires licensure by Tennessee Health Related Boards in Social Work. LCSW preferred
Earned a Master’s degree in social worker or a comparable Master’s level degree from a college or university.
Workplace Experience:
Hospital or Clinical setting required.
Equipment and Skills Training:
Computer-Internet, Meditech and Allscripts Care Management, telephone, printer, fax, copier, language line.
Good Verbal/Written Communication Skills
Physical Environment:
Office and Clinical, possible exposure to hazardous materials, infectious fluids and airborne particles.
Required Weekend Rotation
Physical Effort:
Must be able to walk short distances in hospital environment and patient rooms and possible manual lifting 10-15 lbs, Visual and hearing acuity.
PERFORMANCE STANDARDS
- Provides psychosocial/crisis/ETOH/drug problem assessment, short term counseling and discharge planning to hospital and ER patients who require inpatient/outpatient treatment within 24 hours or next business day of requests. Identifies resources by specialty and insurance contracts and appropriately documents in Meditech/Allscripts regarding assessment of patient’s level of functioning, psychosocial and discharge needs.
- Consistently sets priorities and exhibits efficient time management skills with assigned workload, responding to referrals within 24 hours or next business day of requests with documentation in Meditech within the confinements of scheduled shift.
- Screens patients who do not have consults to identify discharge needs and for the need of post discharge referrals, with 48 hours of admission followed by appropriate documentation in Meditech
- Responds to referrals within 24 hours or next business day of requests with documentation in Meditech Completes assessments within 24 hours or next business day documenting in Meditech/Allscripts addressing the patient's level of functioning, psychosocial and discharge needs communicating clearly accurate information to patient/family.
- Collaborates with members of the healthcare team, patient/family in the development of an appropriate discharge plan, providing the appropriate resources, post discharge option lists, and completion of a post discharge preference form when services are determined. Initiates precertification for those patients needing authorization for post discharge care/transportation.
- Maintains positive working relationships with community agencies, physicians and other members of the healthcare team by communicating clearly timely and accurate information.
- Consistent timely delivery of all appropriate and updated patient information to post discharge providers in Allscripts.
- Communicates Bundle program information to selected patients with the delivery of CMS Bundle letter within 24 hours or next business day of admission.
- Assists Case Manager in patient conversation and delivery of information related to discharge appeals, and payer denials. Actively participates in Discharge Appeal process.
- Enters discharges with correct discharge disposition in Meditech within 3 days post discharge.
