Job Summary
The Care Coordinator plays a vital role in ensuring that patients receive comprehensive and efficient care. This position involves coordinating patient services, managing appointments, and providing administrative support within the Agencies Setting. Duties
- Coordinate client care by scheduling appointments
- Maintain accurate client records and ensure all documentation is completed in compliance with state regulations.
- Provide administrative support by answering phone calls, responding to inquiries, and managing correspondence.
- Assist in the management of office operations to ensure a smooth workflow within the office.
- Educate clients about their care plans and answer any questions they may have regarding their care.
- Collaborate with management to facilitate effective communication and coordination of services for clients.
- Utilize knowledge of medical terminology to assist in the preparation of reports and documentation as required.
Requirements
- Previous experience in an office environment, preferably within a home care agency setting
- Familiarity with medical terminology is essential for effective communication and documentation.
- Strong organizational skills with the ability to manage multiple tasks simultaneously while maintaining attention to detail.
- Excellent interpersonal skills to interact positively with clients, families, and agency employees.
- Proficient in using office software and electronic record systems.
- Ability to work independently as well as part of a team in a fast-paced environment.

Job Type: Full-time
Pay: $18.00 - $19.50 per hour
Expected hours: 36 – 50 per week
