Summary
The Transition Services Teacher assists students with disabilities by providing career assessment, job development and placement assistance, and supports pertaining to maintenance of employment, and self-advocacy. The incumbent in this position will also serve as a liaison between the adult education program, postsecondary education, and community service agencies to provide integrated education and training services (e.g. career assessments, job site development, mentoring, job coaching).

Summary

Essential Duties
- Conducts assessments and advises students on academic and career issues; identifies the educational and career options appropriate for each student and assists with the analysis of each option, including possible outcomes and their implications
- Serves as a District liaison with area businesses, advisory groups, and agencies to encourage the further development of school-to-work opportunities
- Provides training and support to staff members on the integration of the Career Awareness curricula and workplace skills; Develops transition materials and resources
- Collaborates with instructional staff to track student’s progress for academic, career, and personal growth
- Develops ways to link secondary and post-secondary education and new approaches to integrate business with all levels of education
- Provides information to parents regarding service options
- Engages in professional readings for current best practice
- Maintains the flow of communication between agencies and central office
- Works with parents/guardians in all aspects of the student’s educational program
- Facilitates home-school communication by such means as holding conferences, telephoning, and sending written communications
- Uses technology to support instruction
- Serves as a building level resource to faculty, administrators, and related services personnel
- Performs other duties as assigned by the appropriate administrator

- Proven communication and instructional strategies
- Classroom management skills
- Excellent communication skills both orally and in writing
- Excellent organizational skills
- Knowledge of basic first aid methods, techniques for positioning and moving physically challenged students
- Community job-sites
- Ability to lift and carry 30-50 pounds with assistance
- Ability to push or pull a maximum of 50 pounds
- May require stooping or crouching
