Minimum Requirements:
- Must be 18 years of age or older
- High school diploma or equivalent
- Valid driver’s license and reliable transportation
- Ability to pass a background check
- Strong interpersonal and communication skills
- Must be compassionate, patient, and understanding
- Ability to multitask and manage time efficiently
- Must be trustworthy and reliable with the ability to handle confidential information appropriately
- Ability to work effectively in a team environment
- Strong organizational and writing skills
- Additional certifications or training in healthcare or social services is preferred

Essential Functions:
- Provide direct care and assistance with daily living activities such as personal hygiene, meal preparation, and mobility.
- Implement Individual Support Plans (ISPs) and Behavior Support Plans (BSPs) as necessary, including behavior support strategies, therapeutic activities, and skill-building exercises.
- Monitor and document participants’ progress, behaviors, and health status on a daily communication sheet.
- Assist participants in accessing community resources and activities.
- Ensure a safe and supportive environment for all participants.
- Communicate effectively with participants, families, and other team members.
- Administer medications and treatments as prescribed and in accordance with state and federal policies.
- Participate in team meetings and contribute to the development and review of support plans.
- Assist with the onboarding of new participants, including orientation and initial assessments.
- Maintain accurate records and documentation, ensuring compliance with all regulatory requirements.
- Provide transportation using company vehicles for participants to and from community activities and appointments as needed.
